Request for Proposal

If you have viewed our website and/or spoken with a SPARK representative and are ready to bring SPARK to you, please complete this "Request for Proposal" form. Please fill out as much of the form as possible. This will allow a SPARK representative to draft a proposal for you and speed up the process. A SPARK representative will contact you within two (2) business days to go over your "Request for Proposal".

Please note: SPARK needs at least 8 weeks advance notice for dates of workshops. Dates cannot be guaranteed until we receive payment. Please allow enough time to generate a PO or form of payment so SPARK receives it at least 6 weeks before the date of the workshop(s) you are requesting.
