3/18/14: Shape the State

Blue Cross and Blue Shield of Tennessee Foundation

Dedicated to enhancing quality of life in Tennessee, the BCBST Foundation awards grants focused on high-impact initiatives that work with civic and economic partners to promote healthy lifestyles. Apply for a Shape the State grant to transform your Physical Education program with SPARK Middle School PE. The Foundation will award up to 20 grants valued at $10,000 each to Tennessee middle schools as part of its Shape the State program. The grants will be used to purchase SPARK physical education curriculum, training and materials. Applications are due April 18, 2014.

3/13/14: Healthy Kids, Healthy Families

Blue Cross and Blue Shield of Illinois

The Healthy Kids, Healthy Families initiative will invest in and partner with nonprofit organizations that offer sustainable, measurable programs to reach children and their families in the following areas: Nutrition education, Physical activity, Disease prevention and management & Supporting safe environments. 2/20/14: Healthy Generations Grant

WellPoint Foundation

The WellPoint Foundation invests in domestic initiatives that help improve the lives of people and the health of local communities. The Foundation promotes healthy behaviors, health-risk prevention, and healthy environments. Focus areas include childhood obesity prevention. Tax exempt organizations with 501(c)(3) status in (25) select states eligible to apply.

2/13/14: Healthy Communities Small Grant

Wellmark Foundation

The Wellmark Foundation is a non-profit foundation that seeks to improve the health of Iowans, South Dakotans, and their communities. The Foundation provides support to non-profit organizations and public entities to help local communities establish or expand health initiatives. Current funding priorities of the Foundation focus on prevention with these two specific areas: 1) childhood obesity prevention and 2) community-based wellness and prevention.

2/5/14: Champions for Healthy Kids

General Mills Foundation

The General Mills Foundation, in collaboration with the American Dietetic Association Foundation and the President's Council on Physical Fitness and Sports, is accepting applications for its Champions for Healthy Kids grant program. The purpose of the program is to support sustainable, effective programs working to improve nutrition and physical fitness behaviors for youth. Fifty $20,000 grants will be awarded. Non-profit organizations are encouraged to apply.

1/23/14: 2014 ING Unsung Heroes Awards Program

ING

The ING Unsung Heroes program annually provides grants to K-12 educators utilizing new teaching methods and techniques that improve learning. Each year, educators submit applications for an ING Unsung Heroes grant by describing projects they have initiated or would like to pursue. Each year, 100 educators are selected to receive $2,000 to help fund their innovative class projects. Three of those are chosen to receive the top awards of an additional $5,000, $10,000 and $25,000.

1/9/14: Presidential Youth Fitness Program Grant

Presidential Youth Fitness Program, General Mills Foundation

The Presidential Youth Fitness Program is a voluntary program that includes an assessment, professional development, and motivational recognition to empower students to adopt and maintain an active lifestyle. Schools that are selected to receive funding will not receive a check to purchase materials or supplies, but will receive the FITNESSGRAM assessment,training and awards for students. The second round of funding will support schools participating in the Presidential Youth Fitness Program for the 2014-2015 and 2015-2016 school years. Applications will be accepted online from January 1 through January 31, 2014.

12/17/13: Find Your Balance Challenge

Together Counts

The Find Your Balance Challenge is open to elementary school classrooms in grades K-5 and rewards student teams for taking steps toward achieving Energy Balance in your own school communities. Winning schools have a chance to win a $30,000 Grand Prize Grant. One Second Place winning class will receive a $10,000 grant. One Third Place school will receive a $5,000 grant.

11/19/13: PeyBack Foundation Grant

The PeyBack Foundation

The PeyBack Foundation is a nonprofit organization established to assist programs that provide leadership and growth opportunities for at-risk children. Emphasis is placed on programs that have a direct benefit to children through relationships and activities, specifically programs intended to enrich the lives of disadvantaged youth through activities conducted outside the typical school day. Activities include healthy living, mentoring, and leadership skills. Multiple grants averaging $10,000 will be awarded to selected applicants. Nonprofit organizations serving economically disadvantaged youth between ages 6 and 18 in Indiana, Louisiana, Tennessee, and the Denver metro area are eligible to apply.

11/6/13: Teacher Grant Program

Fund for Teachers

The purpose of this funding is to enrich the personal and professional growth of teachers by recognizing and supporting them as they identify and pursue opportunities around the globe that will have the greatest impact on their practice, the academic lives of their students and on their school communities. Grants of up to $5,000 for individuals and up to $10,000 for teams will be awarded to select applicants. Pre-K though grade 12 teachers with at least three years of experience and the intent to return to the classroom the following school year are eligible to apply.

11/1/13: Smart from the Start Grant Program

Healthy Weight Commitment Foundation, National PTA, Discovery Education

With support from the National PTA, schools and communities across the nation have the opportunity to apply for a number of grants and other prizes to help them implement a health initiative at their school. Overall, the Smart from the Start program will award a total of $500,000 in grants, training and awards to eligible schools. The winning preschool will be awarded $20,000 and ten runners-up will each receive $2,500. Early childhood education centers and schools with a Pre-K program are eligible to apply.

National Dairy Council and the National Football League

Funding is available to K-12 schools enrolled in Fuel Up to Play 60. The competitive, nationwide funding program can help your school jumpstart and sustain healthy nutrition and physical activity improvements. Funds can be used to conduct in-school promotions focused on creating a healthier school and to implement Healthy Eating and Physical Activity Plays. Funds can also be used for professional development, nutrition education materials, and physical education equipment & materials.

9/30/13: Healthy Habits for Life

Blue Cross and Blue Shield of Kansas Foundation

The Healthy Habits for Life program is offering a total of $150,000 in grants to assist schools in promoting healthy lifestyle choices to their students. School nurses, physical education teachers, principals, health or family and consumer science teachers, or other administrators are encouraged to apply. The maximum grant amount is $1,000 and grants are limited to one per school. Schools in the BCBSKS Foundation service area, which includes all Kansas counties except Johnson and Wyandotte, are eligible to apply.

Kaiser Permanente Colorado

Kaiser Permanente Colorado is currently offering grants to select school districts to increase physical activity in schools to promote academic achievement. The scope of the work plan in each district will vary based on the amount of funding available and capacity. 9/16/13: Healthy Communities Action Teams to Prevent Childhood Obesity

Virginia Foundation for Healthy Youth

The overall goal of this funding is to improve healthful nutrition and physical activity to prevent and reduce childhood obesity in the Commonwealth of Virginia. VFHY will fund Healthy Communities Action Teams comprised of community groups, partnerships, and coalitions to build and sustain state and local capacity through the implementation of promising practices and activities emphasizing policy, systems and environmental change.

9/9/13: Play60 Super School Contest

National Football League

School Administrators, Teachers, leaders and directors of a PTA and PTO or other school-approved parent organization nationwide are encouraged to submit 3 essays about how their school is celebrating its NFL team pride, about their current PE environment and how their school is teaching the importance of health and wellness. The 34 winning schools will be named NFL PLAY 60 Super Schools and receive an NFL team celebration and a $10,000 NFL PLAY 60 health and wellness grant.

9/5/13: Community Grant Program

CVS Caremark

The CVS Caremark Community Grants Program provides financial assistance to nonprofit organizations that are providing much-needed access to health care for underserved populations; wellness and prevention programs that are aimed at helping people achieve their best health, including programs in public schools; and health education and awareness programs that are helping people on their path to better health. The grant application process for 2013 ends on October 31, 2013 and grants are awarded on a rolling basis. Qualifying organizations are eligible for grants of up to $5,000.

Weingart Foundation

The primary purpose of the Small Grant Program is to increase access to funding and strengthen the capacity of small, community based, and developing organizations. The Weingart Foundation considers applications from organizations working in the areas of health, human services and education. Highest priority is given to organizations or programs that provide greater access for people who are economically disadvantaged and underserved. Priority is given to organizations providing services in the following seven Southern California counties: Los Angeles; Orange; Riverside; San Bernardino; San Diego; Santa Barbara; and Ventura.

The Foundation's Healthy Eating Active Communities program provides funding to build healthy communities in South Florida. 6/17/13: Obesity Prevention Regional Grant Program

The Aetna Foundation

Regional Grants are available for select target cities and states, and focus on healthy food choices and healthy activity. This includes nutrition education and programs that provide opportunities for physical activity in underserved communities. Projects may include school-based or after-school nutrition and fitness programs. Grants will target communities where healthy food can be difficult to buy, and where social and environmental factors may limit people's ability to be physically active. Funding is restricted to nonprofit organizations with 501(c)(3) or similar tax-exempt status and educational institutions. Please see website for the geographic focus for Regional grants. The Baltimore Ravens All Community Team Foundation is committed to improving, encouraging and enabling the healthy development of youth in the Baltimore area, as well as other parts of the state of Maryland. For the seventh consecutive year, the RACTF is executing the Ravens Play 60 Grant which provides funding of up to $5,000 to qualifying nonprofit organizations that create and/or continue programs or projects promoting physical fitness and nutrition education. 4/15/13: 2013 Chargers Champions School Grant Program

The Chargers Champions School Grant program was created to assist San Diego County schools with meeting some of the basic physical fitness, nutrition and athletic needs of their students. The program annually provides selected schools with grants of up to $75,000 for high schools; up to $40,000 for middle schools and up to $30,000 for elementary schools. Previously awarded projects include outdoor fitness trails, weight rooms, running tracks and SPARK equipment. 3/4/13: LibertyGives Foundation

The LibertyGives Foundation supports causes benefiting underprivileged and at-risk youth in Colorado, focused on proactive and preventative services. LibertyGives is currently accepting proposals for funding from $1,000 to $150,000 from public charitable organizations focused on this theme. 2/25/13: Charlotte Martin Foundation Youth Grants

The Charlotte Martin Foundation is a private, independent foundation dedicated to enriching the lives of youth in the areas of athletics, culture, and education and also to preserving and protecting wildlife and habitat. Athletics include a wide range of individual and team sports, with an emphasis on the value of sports for lifelong participation. 2/18/13: Keep Gym in School Grant

In 2012-2013, NFL Network is teaming up with NFL Play 60 and the Super Schools program to offer $10,000 grants to schools in 32 team markets and 2 national markets that show their commitment to health and wellness. 2/13/13: Connections for Cardiovascular Health
AstraZeneca HealthCare Foundation

The Connections for Cardiovascular Health program awards Foundation grants of $150,000 and more annually to US-based, nonprofit 501(c)(3) organizations or similar nonprofit organizations working to improve cardiovascular health in their communities. 2012: Peninsula Health Care District

Community Health Grants Program

The purpose of the Peninsula Health Care District's Community Health Grants Program is to invest in nonprofit, community focused programs, services, and interventions that serve to improve the health status of San Mateo County residents. Program priorities include childhood obesity and nutrition. Nonprofit agencies interested in applying for funding should submit a Letter of Intent describing their proposed project. The Community Health Investment Committee will review the Letters of Intent and determine which organizations will be asked to submit Full Proposals.
